2cupswhole milk
1/2cupheavy cream
1/2cupdark chocolate, chopped
2tbspcocoa powder
2tbspsugar
1tspvanilla extract

Instructions
In a small saucepan, heat the milk and cream over medium heat until warm (do not boil).
Add chopped chocolate, cocoa powder, and sugar. Whisk continuously until melted and smooth.
